Our Founder, Stephanie Cameron, RN, CCM, CAPS
Stephanie is from the Pacific Northwest and has worked in the health care industry for 12+ years. She began her career as a caregiver and quickly discovered her passion for providing care for an aging population. It was a natural progression that she would attend nursing school, and now holds a Registered Nurse degree from Lake Washington Institute of Technology. Stephanie was honored to be chosen by her peers to give the graduation speech.
Stephanie began her care management career immediately after graduating from nursing school and in 2015 she received her CCM (Certified Case Manager) certification. In 2016, she received her Certified Aging in Place Specialist certificate.
She is a member in good standing of the Aging Life Care Association and co-chair of the local unit chapter.
Understanding the need for professional care management services for a quickly expanding aging population, Stephanie founded Transitions Care Management.
In her spare time, Stephanie enjoys photography, playing in the garden, and spending time with her family and pets.
