Our Founder, Stephanie Cameron, RN, CCM, CAPS

Stephanie is from the Pacific Northwest and has worked in the health care industry for 15+ years. She began her career as a caregiver and quickly discovered her passion for providing care for an aging population. It was a natural progression that she would attend nursing school, and now holds a Registered Nurse degree from Lake Washington Institute of Technology. Stephanie was honored to be chosen by her peers to give the graduation speech.

Stephanie began her care management career immediately after graduating from nursing school and in 2015 she received her CCM (Certified Case Manager) certification. In 2016, she received her Certified Aging in Place Specialist certificate.

Understanding the need for professional care management services for a quickly expanding aging population, Stephanie founded Transitions Care Management. She built Transitions Care Management, understanding that each individual is a complex and unique being. She has built a multi-disciplinary team, including nursing, social work, gerontology, and an immensely talented support staff. This combination of truly talented and caring individuals benefit all we serve.

In her spare time, Stephanie enjoys cooking, photography, painting, sculpting, playing in the garden, and spending time with her family and pets.

Stephanie is a member in good standing of the Aging Life Care Association and co-chair of the local unit chapter.

Kelly Rudd, RN, BSN - Project Manager/Director of Marketing/Staffing Specialist

Kelly is a Registered Nurse who has worked in the medical field for over 14 years. Kelly received her Bachelors of Science in Nursing at the University of Washington, where she started her career at University of Washington Hospital. For the past 6 years, Kelly held the position of Branch Manager of the Tacoma and Bellevue offices at Family Resource Home Care, where she worked closely with her clients and their families to coordinate her client’s staffing needs.

Kelly prides herself in carefully and thoughtfully matching caregivers to clients, taking into account the client’s needs as well as client and caregiver personalities. Working with commitment and kindness to provide excellent care and companionship, Kelly has a special talent for creating the perfect client/caregiver match. Those clients of Transitions Care Management that require the services of caregivers will greatly benefit from Kelly’s experience.

Kelly has a high level of understanding with regards to the physical, medical, and psycho-social needs of her clients and works hard to address their issues and needs with respect, integrity, compassion, and professionalism.

Kelly enjoys working with our clients and is always striving to better the lives of our clients so each one of them may have the best quality of life possible.

Kelly has lived in Washington state all her life and in her spare time enjoys the beautiful outdoors (rain and all!), gardening, running, family, and many of the activities our region has to offer.

Ailie Glover, M.A. Gero - Gerontologist Care Manager

Ailie Glover is Transitions Care Management’s Gerontologist Care Manager. Ailie has extensive experience advocating for older adults and their families and helping them navigate their long term care needs. Ailie has worked in a variety of care settings, including skilled nursing, assisted living, life plan communities/CCRCs, memory care, and home care.

Ailie holds a Bachelor’s Degree in Political Science from Washington State University and a Master’s Degree in Gerontology from Georgia State University where her studies focused on dementia care and long term care administration.

Ailie was born and raised in Olympia and is pleased to be back in the Pacific Northwest after spending fourteen years in Atlanta, Georgia. She now lives in Magnolia with her husband and two young children where she enjoys exploring and beach combing in Discovery Park and working in her yard (despite not having a green thumb).

Karly Dickey, RN, BSN - Director of Care Management/Director of Clinical Services

Karly Dickey is our Director of Care Management. She is a Registered Nurse and holds a Bachelors of Science in Nursing from Seattle University.

Karly grew up in Pasadena, CA and first found her love of geriatrics in high school while volunteering at a retirement community. After obtaining her nursing degree, she began work at a Continuing Care Retirement Community in Seattle in their Skilled Nursing. Eight years and a few job promotions later, Karly found herself as the Resident Care Manager at the same community, overseeing their Assisted Living and Memory Support. In addition to nursing skills, she gained valuable knowledge along the way in client advocacy, autonomy and care planning which she brings to her care management work.

Karly enjoys the versatility that care management provides our clients as we navigate their health care journey together. She is honored to work with clients to coordinate and manage their needs so they can focus on aging gracefully.

In her free time, Karly loves spending time outdoors traveling, sailing, or gardening with her husband and their dog Goose. Karly is also an animal foster parent through PAWS, caring for cats and kittens to prepare them for adoption. If you are ever in need of adorable kitten pictures as a pick-me-up, just ask!

Karly is a member in good standing of the Aging Life Care Association.

Paige Selden - Operations Manager

Paige Selden is Transitions Care Management’s Operation Manager, who oversees and helps TCM optimize business practices to ensure efficiency, accountability, and privacy. Paige manages accounting processes, human resources, oversees special projects and is “the glue” of our TCM team. Paige brings years of commercial banking and business operational experience to TCM and is committed to supporting our clients and our Care Managers. Paige grew up in the Chicago suburbs and has been in the Pacific Northwest for 20 years, where she met your husband of 16 years. Paige is an artist who enjoys writing Haiku, gardening, arts & crafts and is currently studying Reiki.

Vicki Balatbat, LPN

Vicki has been serving in healthcare for over ten years. After gaining experience as a CNA, she went to school to become a Licensed Practical Nurse. She has spent the last five years in Oncology, maintaining leadership roles including training nursing students from Seattle University and teaching venipuncture/IV classes to new employees at Seattle Cancer Care Alliance. Since the start of the pandemic, she has been entrusted with the  unique opportunity to manage Covid testing for Seattle’s top professional sports teams, including the Seattle Seahawks, Seattle Mariners, Seattle Storm and Seattle Kraken.
After the personal experience of handling and managing care for a loved one, Vicki’s interested in care management was sparked. Combined with the experience she gained contributing to her own family’s care facility, she is committed to providing optimal care and service to clients and is excited to be a part of the TCM team.

Talia Damian, MA, BA., CMC

Talia is a certified care manager, case manager and mental health specialist with over ten years of experience. As a former educator, she taught third and fifth grade students in public and charter schools. Her career transitioned into private case management services for elderly and special needs clients, and their families. She later joined two fiduciary firms and continued working in this field.

A native of Washington, DC., she spent her formative years in New York, Philadelphia and Los Angeles. An alumna of California State University, Dominguez Hills, she holds a BA degree in Psychology, and a MA in Behavioral Science. Talia is a lifetime member of the Honor Society of Phi Kappa Phi, and Psi Chi-International Honors Society of Psychology.

Her additional certifications and licenses include: Certified Mediator, Los Angeles County Bar Association Dispute Resolution Services Inc.; Professional Fiduciary Management Trust Program of California State University, Fullerton; and Reiki Master.

Talia relocated to Pacific Northwest and is thoroughly in awe of Washington’s beautiful coastline, mountains, sunsets, and wildlife. Talia is a mother of a wonderful daughter who is a culinary school graduate. An avid NBA basketball fan, she enjoys crocheting, listening to live music, succulent gardening, and cooking.

Talia is very passionate about helping others and could not imagine working in another field. She is honored to be a member of the National Academy of Certified Care Managers (NACCM) and the Aging Life Care Association (ALCA). She is also extremely excited to be a part of the Transitions Care Management team!
